The Commission on Higher Education hereby acknowledges the receipt of your application which has been routed to the Student Services Unit. As a matter of procedure, this Office shall conduct documentary analysis of your application to determine completeness and compliance with the requirements of CHED. We shall inform you of the result of the documentary analysis as soon as it is completed.


Documentary Requirements

Academic Requirements
A. For All Applicants:

   1. The printed Application Form from your online application
   2. 2 pcs. 2"x2" ID picture
   3. Certified true copy of Birth Certificate
   4. Parents'/Guardians'Proof of Income (whichever is applicable)
> Latest Income Tax Return (ITR) of parents or guardian
> Certificate of Tax Exemption from the Bureau of Internal Revenue (BIR)
> Certificate of Indigency with Gross Annual Income of parent/guardian issued by the Barangay Chairman or Department of Social Welfare and Development (DSWD)
> Affidavit of No Income or Case Study from DSWD
> For children of Overseas Filipino Workers (OFW) and Seafarers, a latest copy of contract or proof of income may be considered


B. For Incoming Freshmen:

   1. For Graduating Senior High School applicants – Grades for Grade 11 and 1st Semester of Grade 12
   2. For Senior High School Graduates – Form 137


C. Other Applicants

   1. For ALS Passers- Accreditation and Equivalency Test Passer Certificate
   2. For PEPT Passers - Certificate of Advancing to the Next Level


D. For Applicants with single-parent, also submit Solo-Parent ID or Certification from Department of Social Welfare and Development, if applicable
E. For applicants belonging to an Ethnic Group, also submit Certificate from National Commission on Indigenous Peoples (NCIP)
F. For PWD applicants, also submit Medical Certificate
G. For applicants living with a guardian, also submit Affidavit of Guardianship
Procedure:
1. Fill-in the Scholarship Application Form online at https://www.chedro8stufap.com (Application period is from March 1, 2020 to May 31, 2020 only)
2. Print your accomplished application form from your online application.
3. Scan and send the required documents to region8.csp@ched.gov.ph not later than May 31, 2020
Note: The application for financial assistance is completed only after the documentary requirements validation by CHEDRO8. All applications will be subjected to the ranking process in accordance with CHED Guidelines.
